Healthy Solutions for Chronic Conditions
Our Chronic Disease Management programs provide tools and information to assist those with long-term health issues to better manage their conditions. Specific programs are in place for the following high-incidence, high-cost health conditions:

Employees with these conditions will be offered educational materials that emphasize medication compliance and regular physician care. Those with severe conditions are usually offered voluntary one-to-one support from our care managers, who are specially trained in the management of the particular condition.
Healthy Solutions for Complex Care
Our Care Management programs aid your employees in need of complex care. We’ll work with them and their doctors to improve the quality—and effectiveness—of care.
Proactive Care Identification
A program for your employees at risk for a serious medical event. Our care managers will help them navigate today’s complex health care maze. We want them to have the information and services they may need as quickly as possible.
Catastrophic Care Identification
Your employees who have had a traumatic health event, such as a spinal injury, are eligible for support from our care managers. These registered nurses work with the individual’s medical team to help coordinate post-operative care needs. The goal is to facilitate a smooth transition from hospital to home and promote an optimal recovery.
Transplant Care Management
We’ll help coordinate benefits and services for employees before, during and after transplant surgery. If an employee needs an organ or tissue transplant, they’ll need a high-quality medical facility. Our transplant center network includes dozens of accredited facilities across the nation. Each has a strong record of positive, cost effective medical outcomes.
Admission Based Care Management
We proactively identify your hospitalized employees most at risk for readmission to determine whether they might benefit from one-to-one intervention with a nurse care manager. The objective is to help ensure employees’ short-term health needs are met following discharge so that they can avoid readmission. Our ultimate goal is to help your employees return to optimal health.
